i have a table where the user is required to select "yes" or "no" for each item. there would be about 10 items but how will the total number of selected "yes" or "no" appear? also, will the result be recorded?

You can use Checkbox or Radio Button object, or build simple links or buttons with actions attached to every object, which will set some variable's value. Then use something like Submit button with action that calculates result (based on variable's values) and store it to objective using SET SCORE action.
