Creating Certificates with a date and name and scoring of form questions

I am a new courselab user trying to develop my first module. I am trying to create a certificate in courselab which will show the following:
1. Name of person completing the e-learning
2. Score (from questions set up using forms radio buttons)
3. Date e-learning completed.
4. Number of attempts taken on each question and whether they passed it or not

1. I have tried typing in {{name1}} as the variable to pull through the name that will have been input earlier but nothing is pulled through - should I be putting this text in a particular type of box or is there an action required? I am using IE7 would this make a difference?
2. Does anyone know how to insert the date the individual finished the learning?
3. Also, I don't know how to add up the scores from the form questions (radio buttons) does anyone have any instructions re what actions to use on the questions themselves and the summary area? I have been advised not to use the other questions because they will not track in our LMS - ESR's OLM.
